Support Coordinator DDD Program
Job Description:
Support Coordinators assist participants in accessing essential medical, social, educational, and community-based services. Each participant is assigned one Support Coordinator who is responsible for developing and maintaining the Individualized Service Plan (ISP),coordinating with families and providers, and monitoring service delivery.
Qualifications:
Bachelor’s degree in any field
At least 1 year of experience working with adults (21+) with developmental disabilities
Experience may include paid work, caregiving, or relevant volunteer service
Must pass state, federal, and central registry background checks
Completion of all required DDD trainings prior to service delivery
Support Coordination Supervisors must meet all Support Coordinator qualifications and may not supervise plans for family members.
Job Coach – DVR Employment Services
Job Description:
Job Coaches provide hands-on support to individuals receiving services through the Division of Vocational Rehabilitation Services (DVRS). They assist participants in learning job tasks, adjusting to workplace environments, and developing the skills needed to succeed in their roles. Job Coaches also help with travel training, on-the-job support, and communication with employers to ensure long-term employment success.
Qualifications:
High school diploma or equivalent
At least 1 year of experience supporting individuals with disabilities in employment or community-based settings
Strong communication and problem-solving skills
Ability to work independently and travel to various job sites as needed
Must pass state and federal background checks as required by DVR
